Certified Medical Assistant- Chronic Care Management Fast-growing Telehealth provider is looking for an experienced Medical Assistant. Qualifications: ? CMA or RMA certification (Required) ? As a Care Manager you would be working with a group of patients over the phone providing teaching, coaching, and motivation to help them improve their chronic conditions. Responsibilities Include: ? Triaging patient calls ? Conduct regular condition assessments ? Patient-Centered Care Plans ? Monthly med reconciliations ? Patient Education ? Contact with providers as needed Qualifications: ? CMA or RMA License ? Knowledge of third-party payer requirements ? Familiarity with electronic medical records ? Strong aptitude for detail and ability to work independently ?
