Job Description: Clinical Pharmacist – Psychiatric Medication Management (Los Angeles, CA) Position Type: Full-Time or Part-Time (Flexible) Mostly Remote Location: Los Angeles, CA 90003 Reports To: Supervising Psychiatrist / Medical Director About the Role We are seeking a Clinical Pharmacist to join our behavioral health care team in Los Angeles. In collaboration with a supervising psychiatrist, the pharmacist will play a central role in medication management services for patients with psychiatric and co-occurring conditions. This role is designed to improve access, continuity, and quality of care while reducing reliance on high-cost prescriber hours. Key Responsibilities • Conduct comprehensive medication therapy management (MTM), including reconciliation, adherence assessments, and drug interaction reviews. • Monitor patients’ medication response, side effects, and lab values. • Provide refill management and dosage adjustments under a Collaborative Practice Agreement (CPA) with the supervising psychiatrist, in alignment with California regulations. • Educate patients and families on medication use, side effects, and adherence strategies. • Document clinical encounters and communicate changes promptly with the psychiatrist and care team. • Participate in interdisciplinary team meetings, contributing expertise in pharmacology and medication safety. • Support transitions of care by ensuring medication continuity after hospital discharge or program changes. • Serve as a resource to staff regarding psychotropic medication protocols and best practices. Qualifications • Doctor of Pharmacy (PharmD) degree required. • Active California pharmacist license in good standing. • Experience in behavioral health, psychiatry, or primary care preferred. • Strong knowledge of psychotropic medications and medication safety. • Excellent communication, counseling, and patient engagement skills. • Ability to work collaboratively with psychiatrists, clinicians, and support staff. Preferred Skills • Prior experience working under a Collaborative Practice Agreement (CPA). • Familiarity with Medi-Cal, Medicare, or FQHC/CCBHC clinical settings. • Bilingual abilities (Spanish/English) highly valued in Los Angeles patient populations.
